We make health equity happen.
Our Current Projects
The Community Health Worker (CHW) Initiative
Launched in September 2024, CCHI’s Community Health Worker (CHW) Program is designed to strengthen the local health workforce and expand access to care across California.
In partnership with Partnership HealthPlan of California, we train and certify CHWs with the skills and tools they need to support their communities.
Through collaboration with our longtime member organization, California Community Health Initiative (CalCHI), we also provide CHW services and help our partner bill for these essential services.
Connecting Kids to Coverage Outreach and Enrollment Cooperative Agreements (CKC) 2025
Since August 2025, CCHI has worked with 6 amazing subcontractors in 7 California counties under the CMS 2025 CKC Grant.
This initiative focuses on providing Outreach, Enrollment, Retention, and Utilization (OERU) services for children, parents, and pregnant individuals to enroll into Medi-Cal, or the Children’s Health Insurance Program (CHIP).
The California Health Interconnectivity Initiative (CHII)
Led by CCHI, the CHII is a statewide effort to strengthen collaboration across California’s safety-net. The initiative connects local governments, community-based organizations, health plans, and clinics to improve coordination, data sharing, and equitable access to care.
Guided by Herb K. Schultz, former Regional Director for the U.S. Department of Health and Human Services, CHII convenes seven regional coalitions and an annual statewide forum to identify shared challenges, lift up local innovations, and shape collective solutions.
CCHI will produce a statewide report in 2026 summarizing key findings, best practices, and recommendations to inform future cross-sector partnerships and funding strategies.
Funded by The California Wellness Foundation, this work builds the foundation for a more connected, equitable, and resilient health ecosystem across California.
Our Past Projects

Additional Initiatives
Advocacy & Policy
Annually, CCHI brings together our member organizations and health advocates across the state to elevate their community’s voices at our Day at the Capitol.
Through legislative visits and coalition-building, CCHI and its members advocate for policies that protect and expand access to health care for all.
Our advocacy efforts are grounded in lived experience and driven by the belief that the communities most impacted by health inequities should lead the conversation for change.
Health Education
CCHI is committed to keeping our members informed, engaged, and equipped to support their communities.
Through Health Access Assister (HAA) Forums, statewide membership meetings, and timely resource sharing, we provide education on health policy, program updates, and best practices in enrollment and outreach.
Our educational initiatives ensure that front-line staff and organizational leaders alike have the tools they need to navigate California’s evolving health care landscape.
CCHI would like to thank its current and former funders for their support, generosity, and guidance.
Your contributions have impacted millions and allowed CCHI and its members to continue advancing health equity for all Californians!
